Cesare Massaro
Cesare is the don’s nephew and Enzo’s friend, played by Christian Antidormi. The PlayStation blurb calls him charming and insecure, hiding doubt behind bravado, and hungry to inherit his uncle’s place.

What I checked
The spoiler is the search query. Cesare kills Enzo. The order is: Enzo spares Cesare in the knife fight, Enzo mortally wounds Torrisi, Cesare stabs Enzo while Etna erupts. I will not call that simple cruelty. The oath reading — Cesare still believes the vow after Enzo breaks it by killing the don — comes from the synopsis plus player discussion. Hangar 13 has not, in the sources I have, published a line that confirms the psychology. Treat it as a reading that fits the blurb.
Three ways to say why
The plot reason: Enzo killed the head of the family. The character reason in the official blurb: Cesare wants to be worthy of that family. The player reason you will see repeated: someone had to enforce the oath, and Cesare decided it was him.

Enzo killed Don Torrisi. Cesare answers that with a knife.
