The Mafia series, and this game
The Old Country is the 2025 game set in 1904–1907 Sicily. It is a prequel. The publisher does not call it Mafia 4.

What I checked
I used the series histories cited in the research log for the years, and the game’s own store page for the 2025 date. People searching “mafia wiki” often want this map of titles more than a plot dump. The other games stay one sentence each so this page does not pretend to be their guide.
The numbered confusion
There are two different “fourth” counts. One count says The Old Country is the fourth original story. Another count, used for the 2020 remake, has called that remake a fourth release. I write both and I do not pick a slogan.
