What I checked
Search engines still cluster the phrases because this is the next original story after Mafia III. That is a counting habit, not a logo. Mafia IV, as a Roman-numeral rumor, is a different pile of posts and should not be treated as a subtitle I found on a store page. I checked the store name again through the Steam API on 2026-10-22. The name field is Mafia: The Old Country.
Why people still say 4
If you count original stories — 2002, 2010, 2016, 2025 — this is the fourth. If you count the 2020 remake as an entry, the word fourth was already used for that remake. Both sentences can be true, and neither one is printed on the box.
