Do you need the other games first?
No. The Old Country stands on its own. It is set decades before the 2002 game. You will miss a few returning names, not the plot.

What I checked
I get this question because Leo Galante, Frank Vinci, and Sam Trapani’s father are in the orbit of later games. You can enjoy Enzo’s story without those echoes. If you want the echoes afterward, play the 2020 Definitive Edition of the first game, then Mafia II, then Mafia III. That is a recommendation, not a requirement Hangar 13 printed as a warning.
If you want a sequence anyway
Start with The Old Country if Sicily is the reason you are here. Start with the 2020 remake if you want the 1930s story people mean by “the first Mafia.” Do not start with Mafia III if what you liked was a short, directed campaign.

FAQ
Will I be lost?
No. The Torrisi story explains itself.
Where should I go after the credits?
