Trophy guide for Mafia: The Old Country
The PS5 list is 51 trophies: one platinum, two gold, ten silver, and 38 bronze. PowerPyx rates it 4/10 and about 15–20 hours on a Hard playthrough.

What I checked
I am following PowerPyx’s roadmap for the structure and flagging the Steam conflict. The Steam store page displays 62 achievements. PSN is 51, plus 7 in the DLC, which is 58, not 62. I do not know what the extra Steam rows are, so I will not pretend the lists match. There is no new game plus. There is one autosave. The missable is Salvatore’s Apprentice: if you collect every note that contains a safe code before the trophy can pop, chapter replay will not hand it to you.
A clean route
Play the whole story on Hard if you want True Soldato. Switching only the last chapter does not count. Most collectibles can be finished in Free Ride or chapter replay. The fox purchase at 5,000 Dinari was bugged if you were short on money; the September 22, 2025 patch fixed that stall.
Kill routing: chapter 11 enemies drop every weapon type, which is the published farming spot. Money routing: two blue crates at the start of chapter 7, then restart the checkpoint. I am passing those on as PowerPyx’s notes, not as a route I timed.
| Rule | What it means |
|---|---|
| Hard | Entire playthrough for True Soldato |
| Missable | Salvatore's Apprentice |
| Saves | One autosave, no manual slot |
| NG+ | None |
| DLC | 7 more trophies, separate list |
