Is Mafia: The Old Country on PS4?
No. Mafia: The Old Country does not have a PS4 version. The PlayStation store page is a PS5 game.

What I checked
I looked for a PS4 SKU on the US PlayStation page and in the platform notes used for this guide. PowerPyx also says there is no PS4 version. I am not going to suggest a workaround build, because none is sold.
If you only have a PS4
You need a PS5, an Xbox Series X|S, or a Windows PC that meets the Steam minimum. Remote Play, where the PS5 page lists it, streams from a PS5. It does not install the game on a PS4.
Do not buy a listing that claims a PS4 download. The supported set is PS5, Series X|S, and Windows.
