Where the story is set

The game is set in Sicily in the early 1900s, specifically 1904 to 1907. The region you live in is called Valle Dorata. The town is San Celeste, a name that also appears in Mafia II.

What I checked

I used the 2K developer diary for the real-world research and the wiki location list for the fictional map. Hangar 13 and Stormind Games scouted Sicily: countryside, coast, ruins, villas, a knife-maker’s shop, horses and cars in the same decade. I am not going to say the game includes Scala dei Turchi. That cliff is real, and it is not on the location list I checked. Searches that pair the Italian title with that landmark are not proof of an in-game site.

Fictional places

Towns in the location list: Collezolfo, Fiumansa, Porto Almaro, and San Celeste. Palermo is chapter 12, La Forza Del Destino. The sulfur mine at Collezolfo is where Enzo’s story starts.
